Purpose of Position: 

 

Under the direction of the Manager of Clinical Services, Behavioral Health Services, and in conjunction with the on-call supervising psychiatrist, this position is responsible for providing immediate, on-site psychiatric assessments, psychopharmacological and level of care recommendations for individuals of all ages (depending on the scope of practice) presenting with behavioral health crises.  As an APRN, this Clinician prescribes medication, implements patient teaching and facilitates next-care linkages, referrals and/or admissions, to ensure smooth and timely transitions to appropriate level of care/service.

 

Major Accountabilities:

 

1. Conduct psychiatric assessments of individuals presenting in crisis utilizing BHIS psychiatric evaluation standards/protocols and tools.  Document all findings/contacts in client's record in accordance with established medical record guidelines.

 

2. Communicate findings and recommendations to other staff as indicated and consult with on-call supervising psychiatrist as needed.  Facilitate timely communications with third party payers and outside providers regarding level of care assessments and authorizations when needed.

 

3. Completes medication orders and prescriptions, diagnostic studies, and admission orders.  Facilitate medical clearance process as indicated.

 

4.  Respond to psychiatric emergencies utilizing training in management of aggressive behaviors. Demonstrate skills and initiative to prevent situations from escalating by recognizing potential at-risk situations and de-escalating clients. 

 

5. Complete and facilitate prescriptions and referrals and/or admissions to indicated level of care, programs, and agencies.  Utilize a case management approach to monitor client compliance with plan and appropriateness of referrals. Intervene, as necessary, to modify plan of care.

 

6. Provide psycho education and medication teaching to clients and families documenting appropriately.  Act as client advocate; inform client/family of available community and government resources.

 

7. Maintain current knowledge of community resources, BHIS programs, care management strategies, level of care criteria, regulatory agencies and applicable legal statutes.

 

8. Establish and maintain good rapport and working relationships with colleagues in various settings to foster the communications necessary for the delivery of quality patient care.  Serve as a psychiatric clinical resource and act as a liaison between other programs/providers and BHIS programming.

 

9. Participate in quality improvement initiatives, utilization management activities, clinical/administrative staff meetings and assigned committees.  Participate in training and development activities for the purpose of professional growth and skill enhancement.

 

10. Follow clinical and administrative policies and procedures.

 

11. Maintain patient rights and confidentiality.

 

12. Perform other duties as directed.

 

Education/Experience/Qualifications:

·         Currently licensed in MA as Registered Nurse. 

·         ANA Certification as a Psychiatric Clinical Nurse Specialist or Nurse Practitioner preferred.

·         US Department of Justice and Massachusetts' Controlled Substance DEA Registrations are required for prescribing practitioners. 

·         Crisis Prevention Intervention Training within 3 months of new position for any practitioner working in the In-Patient Psych Department.

·         Master's Degree.

·         One to two years experience.

·         Demonstrated ability to work as part of a team.

·         Advanced diagnostic skills.

·         Ability to clearly summarize pertinent clinical information.

·         Case management skills.

·         Crisis intervention skills.

·         Thorough knowledge of psychopharmacology.

·         Knowledge of addictive disease assessment and treatment.
