Job Details

Provides psycho-social assessments, crisis counseling and discharge planning to Falmouth Hospital patients and their families recognizing age specific differences in assessing the needs of the patients.
  • Assists patients and families to adjust to illness and lifestyle changes related to illness and hospitalization.
  • Interviews patient and family members to obtain information about home environment, family relationships, health history, and patient's personality traits.
  • Evaluates the ongoing medical and psychosocial needs for home care planning in order to set up appropriate community services.
  • Evaluates data gathered in terms of the medical plan of treatment, available social service programs and plans a pertinent psychosocial treatment program that will provide maximum benefit for each patient.
  • Assists patient/family in the procurement of essential equipment to allow optimum and safe function at home.
  • Assists patient/family in the decision making process leading to the selection of and placement in an appropriate long term care facility.
  • Provides information and referral for continuing care and crisis intervention.
  • Functions as a liaison on behalf of patients with appropriate community agencies.
  • Provides services which comply with: Public Health, JCAHO, Medicaid, Medicare, HMO's and PRO regulations. Screens all admissions to the hospital to determine whether discharge planning services may be needed.
  • Provides therapeutic short term counseling to patients and families.
  • Communicates diagnoses and activities of daily living status to nursing home staff.
  • Provides mental health and substance abuse assessments of Emergency Department patients, recognizing each patients requirements relative to their age-specific needs.
  • Works with psychiatrists/APRN in assessing and treating psychiatric patients.
  • Makes referrals and secures placement for inpatient psychiatric care and/or outpatient mental health treatment.
  • Assists in the coordination of child abuse and elder abuse procedures and mandated reporting.
  • Coordinates and leads patient focused conferences with physicians, nurses and other staff when the need arises.
  • Provides crisis intervention during covered hours.
  • Telephone follow-up of selected discharge patients, as required by the DPH.
  • Provides training and supervision, as appropriate, for social work students as part of their field instruction.
  • Participates in the development of in-service programs for hospital personnel addressing the psychosocial needs of patients and families.
  • Prepares and submits monthly statistical and quality assurance reports.
  • Assists in fulfilling audits and other federal, state, and JCAHO requirements.
  • Participate in screening patients for admissions into agency programs, utilizing pre-approved admission criteria.
  • Prepare written reports of assessment results and prepare recommendations for the level of care and treatment needs; including disposition recommendations.
  • Evaluate patient needs and advocate the least restrictive level of care consistent with the defined patient needs.
  • Coordinates the flow of information including the preparation of the Patient Care referral form between the hospital and the other facilities and agencies.
  • Utilizes discretion regarding the appropriate protection and/or use of confident information.
  • Contacts managed care representatives, third party payers, primary physicians or other relevant persons in order to provide clinical and/or demographic information required to verify insurance and/or obtain authorization for clinical services.
  • Triages all incoming calls from either internal or external referral sources.
  • Places calls to outside facilities when the inpatient unit or PHP does not have available beds or the patient does not meet level of care.
  • Performs other work related duties as requested.

  • Master's degree in Social Work (MSW) and State licensure (L.I.C.S.W.)
  • Minimum of three years experience in an Emergency Center setting including Crisis Intervention, Psychosocials, Referrals.
  • Minimum of three years experience working with Behavioral Health patients, ED patients and families in an Emergency Center setting.
  • Familiarity with local and state community resources and ability to access same.
  • CPI (Crisis Prevention) within 4 months of new position and must renew annually.
  • Ability to work in a demanding and sometimes dangerous environment.
  • Ability to read, write and communicate in English
