Hospice RN After Hours Triage Coord-180327-50007721
Description Responsible for coordinating skilled nursing intervention and supportive care to patientsâ and contracted skilled nursing facilities, for weekend, holiday, evening and night on-call duty, in accordance with the Plan of Care for each patient.

Respond timely to all calls received from answering service. Triage calls from MDâs, staff, patients, caregivers, and families.

Assigns and/or directs nurses when appropriate.

Coach, support, and monitor progress of after hours visit nurse.

Initiate communications with attending physicians and other community resources to report changes and coordinate optimal care and use of resources for the patient/family.

Obtain data on physical, psychological, social and spiritual factors that may influence patient/family health status and incorporate that data into the plan of care as indicated.

Maintain up-to-date patient records so that problems, plans, actions and goals are accurately and clearly stated and changes are reflected as they occur. In a timely manner, documents in the patient record any call, problem, solution and disposition.

Monitor physical care of the patient by instructing primary caregivers, volunteers and employed caregivers.

Leave any changes to the plan of care on the Hospice report line.

Respond to medical or emotional crisis and/or death of patient and schedule home visit to provide direct care admission or nurse pronouncement as appropriate.

Attend interdisciplinary team/staff meetings whenever possible.

Provide appropriate counsel at the time of death. Report death and any bereavement risk factors to bereavement coordinator.

All other duties as assigned.
Qualifications:

Current RN license in the state of Massachusetts.

Minimum of two (2) years of recent varied work experience as a professional nurse in oncology, home health and hospice setting.

Hospice experience.
-
Triage experience.
Schedule Details: Per Diem overnight, phone triage 4pm 8am, Mon-Fri
