Job Details

The primary purpose of your job position is to assist residents in activities of daily living and interventions as appropriate to meet the needs of the participants in accordance with current applicable Heritage Federal State and Local standards guidelines and regulations our established policies and procedures
  • Is knowledgeable of multiple department practices, procedures, safety regulations, and guidelines.
  • Assists residents in activities of daily living including dressing, toileting, bathing, grooming and oral hygiene in a professional manner.
  • Demonstrates patience and understanding with residents at all times.
  • Checks on and responds to the resident’s need to ensure safety and comfort.
  • Provides services for residents in a professional and efficient manner.
  • Responds to E-CALLs immediately, regardless of assignment.
  • Receives and gives reports of on-coming and out-going shifts by oral and written log.
  • Informs supervisor of changes in the resident’s condition and needs. Documents these observations thoroughly and communicates any changes to staff.
  • Uses assessment capabilities to determine for residents requiring additional help.
  • Promptly reports allegations of abuse or neglect to Executive Director.
  • Maintains a positive and professional environment and projects a calm and competent demeanor.
  • Responds to residents, families, visitors and guests in a professional, respectful and dignifies manner.
  • Follows plan of service for residents set by management team.
  • Documents information in a clear, concise, accurate and professional manner.
  • Makes accurate entries onto ADL sheets and charts according to Heritage policy and procedure.
  • Informs, encourages and assists residents to attend scheduled activities.
  • Performs dining room duties as directed.
  • Adheres to infection control procedures.
  • Recommends need for additional services for residents to the management staff.
  • Reports all unusual incidents to the management staff and all staff as appropriate.
  • Follows state regulation for requirement for in-service hours per year.
  • Strictly follows SAMM policies and procedures.
  • Knows and complies with Resident’s Bill of Rights.
  • Understands and adheres to established fire, disaster, emergency and employee safety programs.
  • Empties trash, washes dishes, and performs other minor housekeeping functions as needed in the resident apartments and commons areas.
  • Makes up beds, exercising proper precautions in handling of soiled linens.
  • Assists and maintains a neat and obstacle free environment throughout Heritage.
  • Attends team meetings and offers input to the management team.
  • Participates in in-service training programs for PCAs and other related services as outlined in our in-service training policies and procedures.
  • Receives, follows and ensures that schedules are followed as outlined by supervisor.
  • Answers the phone in a prompt, friendly and professional manner.
  • Maintains the privacy of both the residents, visitors and staff.
  • Handles messages accurately, completely and promptly.
  • Efficiently and accurately forwards all calls to appropriate department.
  • Informs appropriate staff about deliveries.
  • Records and files as required.
  • Coordinates schedules and makes appointments.
  • Prepares correspondence and other printed materials.
  • Maintains confidentiality of records.
  • Compiles and maintains Resident Care Department records.
  • Organizes files and maintains files systems and correspondence.
  • Conducts research, compiles and types statistical reports.
  • Performs other clerical duties as needed, such as filing, photocopying and collating.
  • Assists management with follow up on resident needs.
  • Trains PCA staff as directed.
  • Participates in Heritage work groups, i.e., performance improvement committee, safety committee, etc.
  • Performs activities, as directed, in accordance with established procedures.
  • Cleans common areas as directed.
  • Reports to manager whenever supplies need to be ordered.
  • Performs unscheduled tasks as directed.
  • Reports all accidents/incidents to your supervisor, no matter how minor they may be. (NOTE: Such occurrences must be reported on the shift on which they occur).
  • Agrees not to disclose assigned user ID code and Password for accessing resident/facility information and promptly report suspected or known violations of such disclosure to the Executive Director.
  • Inspects areas on a daily basis to ensure that the grounds, buildings and equipment are maintained in accordance with established policies and procedure and all hazardous areas are properly identified.
  • Assures that the common areas and equipment are properly maintained for comfort, convenience and safety, operating to manufacturer’s specifications.
  • Assists in the safety and security of Heritage.
  • Assists with keeping dining room and hallways clear of assistive devices during meals and as needed.
  • Maintains appropriate records as directed.
  • Follows programs/activities as necessary/approved.
  • Reports and assists in identifying and correcting problem areas and equipment, and/or improvement of services, when applicable.
  • Performs other related duties and activities as may become necessary or as directed supervisor.
  • Reports and maintains an adequate level of required supplies and equipment.
  • Follows polices that will effectively maintain programs and operations for Heritage.
  • Completes necessary forms, reports, etc., submitting such to supervisor as required.
  • Develops and maintains a good working rapport with inter-departmental personnel, and other departments within Heritage, to assure that programs can be properly planned and maintained to meet the needs of Heritage.
  • Participates in programs designed for in-service education, on the job training and orientation classes.
  • Adheres to department working hours and completes assignments within guidelines set by supervisor.
  • Adheres to Universal Precautions and Infection Control Policies, including following established hand washing procedures, use of appropriate PPE and supplies when handling infectious materials.
  • Follows established policies governing the use/disposal of PPE and disposal of infectious wastes.
  • Disposes of refuse/trash daily in accordance with our established sanitation procedures.
  • Has a competent understanding of all chemicals and has been trained to use them in accordance with standards.
  • Wear and/or use safety equipment and supplies and uses proper mechanics while performing duties (e.g., back brace, mechanical lifts, etc. when moving or lifting heavy objects).
  • Keeps work area(s) free of hazardous conditions, i.e., grease/oil spills, excess supplies, equipment, etc., to assure that all established safety rules/regulations are followed at all times. Refers to materials safety when necessary.
  • Follows established policies governing the use of labels and MSDSs.
  • Handles emergency situations in a calm and efficient manner.
  • Ensures that work/assignment areas are clean and equipment, tools, supplies, etc., are properly stored before leaving such areas on breaks, end of work day, etc.

  • High School degree.
  • 1 to 5 years experience.
  • Must be certified CNA or HHA

Schedule Details: 32 hours a week, day and evening shifts, rotating weekends, rotating holidays
