Provide admission assessments and aftercare planning and follow up for acute care services(Inpatient PHP Crisis). Responsible for providing Social Work UR coverage for Inpatient and Partial Hospitalization Programs.
  • In coordination with the admitting or on call psychiatrist conduct initial and or level of care clinical assessments of individuals presenting with acute care needs
  • Prepare an Initial Treatment Plan based on the findings from the assessment and communicate the initial assessment findings and treatment plan to multidisciplinary teams
  • Conduct group individual and family case management meetings and other related group sessions
  • Assist in Discharge Planning for patients receiving acute care services and provide aftercare management and follow up
  • Communicate and collaborate with the patients families and aftercare providers in developing the plan
  • Actively monitor and promote patient s aftercare plans providing support as needed
  • Initiate crisis prevention and crisis intervention services as needed to newly discharged patients in coordination with the patient's family and primary caregivers Interventions may be telephonic or in person
  • Revise the patient s aftercare plan as needed to accommodate any changes in the patient s condition or circumstances following discharge
  • Provide fill in coverage for the Inpatient and Partial Hospital Program Social Workers during their absence due to vacation sickness etc Perform the duties of the Psychiatric Social Worker
  • Provide fill in coverage for the Pyschiatric Utilization Review Coordinator during absences due to vacation sickness etc Perform duties of Psychiatric Utilization Review Coordinator
  • Attend continuing education programs specific to providing behavioral health care to different population groups and diagnostic categories
  • Assist with community relations activities including community contacts and education
  • Perform other work related duties as requested directed

  • Masters degree in Social Work from a CSWE accredited institution
  • Massachusetts licensure as a Licensed Independent Clinical Social Worker (LICSW)
  • A minimum of two years recent (last 5 years) experience working in one of the following structured multidisciplinary mental health settings Inpatient Partial Hospital
  • Crisis Stabilization Program or Day Treatment Crisis Prevention Intervention Training within 3 months of new position

Schedule Details: Per diem, 8 hour shifts, rotating days, occasional weekends and holidays
