We are currently seeking a dedicated Social Worker to join our team at Cape Cod Hospital.

Plans, organizes and implements social work services to patients and families. Plans for discharge and aftercare. Maintains contact with referring agencies and related people, and with community resources used for referral




1. Provide clinical social work services on a day to day basis to the Partial

Hospitalization Program including psychosocial assessments, group work,

family assessment and work, and discharge planning. (10%) (E)


2. Assume primary responsibility for case management of each patient in the

Partial Hospitalization Program. Participate in the implementation of the

treatment plan. Maintain client records. (10%) (E)


3. Participate, with other members of the treatment team, in periodic review of

the treatment plan and make for necessary modifications or adjustments.

4. Conduct group, individual and family case management meetings and conduct

other related group sessions. (10%) (E)


5. Coordinate the various services provided by the program itself, and by

other agencies. Meet regularly with relatives and significant friends and

others, unless clinically contra-indicated. (10%) (E)


6. Monitor the client's progress in accomplishing the treatment goals. (10%)(E)


7. Ensure that staff review the client's overall progress every thirty

attendance days, with the participation of the client and alter and revise

treatment plan, as necessary. (10%) (E)


8. Ensure that patient maintains all follow-up appointments and adequately

participates in the treatment plan. Document all such efforts in the

client's record. (10%) (E)


9. Ensure that a periodic review of the client's case to ensure all thera-

peutic goals have been met or that the patient has ceased to substantially

benefit from the program. (10%) (E)

10. Directly, or through referral arrangements, ensure the development of

aftercare plans for all discharged clients to ensure that their long-term

needs are met. (5%) (E)


11. Perform related marketing and other activities for the Cape Psych Center

Programs. Transport patients in CPC van. Perform other work related

duties as requested/directed. (5%) (E)




· Master's degree in Social Work (MSW) and State licensure (L.I.C.S.W.)

· CPI (Crisis Prevention) within 4 months of new position and must renew annually.

· Ability to read, write and communicate in English.
