Plans, organizes and implements social work services to patients and families in the emergency center. Plans for discharge and aftercare. Maintains contact with referring agencies and related people, and with community resources used for referral.
  • Assists patients and families to adjust to illness and lifestyle changes related to illness and hospitalization.
  • Remains available to patients, family and staff through an open referral system. Interviews patient and family members to obtain information about home environment, family relationships, health history, and patient's personality traits. Assists patients and families to understand, accept, and follow medical recommendations.
  • Utilizes resources to assist patient to resume life in the community or to learn to live with disability.
  • Assists patients and family in understanding and negotiating with third party payors as well as other community services.
  • Assists in patient care planning to ensure continuation of treatment in an atmosphere conducive to preservation of health.
  • Evaluates the ongoing medical and psychosocial needs for home care planning in order to set up appropriate community services.
  • Evaluates data gathered in terms of the medical plan of treatment, available social service programs and plans a pertinent psychosocial treatment program that will provide maximum benefit for each patient.
  • Assists patient/family in the procurement of essential equipment to allow optimum and safe function at home.
  • Assists patient/family in the decision making process leading to the selection of and placement in an appropriate long term and short term facilities.
  • Performs a variety of services such as advising on social problems, arranging for discharge or post-operative care at home or in institutions, helping to alleviate level of anxiety and fear.
  • Screens patients presenting with emotional, mental health and substance abuse issues and coordinates patients care and discharge.
  • Assists families of the Emergency Center patient in locating patient and understanding patient care plans as needed.
  • Provides information and referral for continuing care and crisis intervention. Ability to work within an Emergency Center case management system.
  • Brings voluntary community bases services into play as appropriate to patient and family needs Utilizes resources to assist patient to resume life in the community or to learn to live with disability.
  • Functions as a liaison on behalf of patients with appropriate community agencies.
  • Evaluates the appropriateness of community services as possible and develops plans to meet patient needs.
  • Maintains excellent relationships with community agencies.
  • Functions within the professional Social Work Code of Ethics Assists in management of Emergency Center prescription and taxi budget.
  • Acts in accordance with professional ethics and within departmental policy and procedure.
  • Be available to all clinical areas for social work services as needed and assigned.
  • Prepares and updates records for each patient, documenting as per policy. Maintains monthly statistical data.
  • Observes all safety, infection and environmental control procedures.
  • Functions in such a way as to meet departmental quality assurance program. Assists in care and maintenance of department equipment and supplies. Attends and actively participates in meetings and serves on committees as requested.
  • Works to promote a positive perception of CCH Emergency Center by the community.
  • Performs other work related duties as assigned or requested.

  • Master's degree in Social Work (MSW) or related discipline and State licensure (L.C.S.W.)
  • Significant experience in an Emergency Center setting.
  • Significant crisis intervention training and experience.
  • Ability to work in a demanding and sometimes dangerous environment.
  • Familiarity with community resources and ability to access same.
  • Significant experience in working with patient and families in an Emergency Center environment.
  • CPI (Crisis Prevention) within 4 months of new position and must renew annually.
