The Team Leader has the responsibility for direct oversight of the clinician to maintain the quality and safety of patient care utilizing evidence based practice.
  • In collaboration with the Human Resources Department reviews application, interview and hire employees.
  • Assist with the orientation and preceptor program for all new direct reports.
  • Coach, support, and monitor progress of all direct reports.
  • Educate and evaluate staff in case management responsibilities and in the utilization of evidence based practice through caseload review.
  • Teach staff how to document to be compliant with agency and regulatory standards.
  • Ensure financial stability by teaching and evaluating clinicians correct OASIS documentation as well as proper utilization of other disciplines and resources.
  • Ensure staff are meeting productivity.
  • Demonstrate the ability to empower self and staff for quality patient outcomes.
  • Use clinical theory to problem solve, evaluate clinician compliance to standards, and assess quality assurance using improvement processes.
  • Provide clinical expertise and serve as a resource to staff (including CHHA’s) and community.
  • Conduct team meetings and interdisciplinary case conferences.
  • Routinely rotates through clinical management on-call responsibilities to ensure proper patient/staff safety.
  • Ensure weekend/holiday on-call and staff coverage to meet agency need.
  • Provide direct care as indicated by agency needs.
  • Assume delegated responsibilities in the absence of the clinical manager.
  • Complete performance evaluations in a timely manner at 4 month, annual or as needed.
  • Work in collaboration with Human Resources to discipline clinicians through education, corrective action plans, verbal/written warnings and or discharge as indicated.
  • All other duties as assigned.

  • Current license in MA in field of expertise.
  • CPR is required.
  • Minimum three (3) years clinical experience including two (2) in homecare/community health required.
  • BS or Masters in field of expertise preferred.
  • Computer skills preferred.

Schedule Details: 37.5 hours per week, days, weekend and holiday rotation
